Mint Health offers a Hybrid Membership Model of Care!
Fully incorporating Integrative Functional Wellness visits across our practice, we are able to take a more thorough and more effective approach to lifelong health and preventive health, as well as addressing difficult/chronic conditions. Integrative Medicine is a partnership of care that involves the full range of physical, emotional, social, and environmental influences that affect a person’s health, and may use interventions from an array of scientific disciplines to guide individuals towards optimal health.
We now offer One Fully Comprehensive Functional Medicine Membership - the Mint Health Membership
General Information:
All office visit charges are billed to the patient’s insurance. Patients are responsible for any applicable co-payments, co-insurance and deductible amounts with our providers throughout their membership.
Services offered under the memberships are beyond coverage of any insurance plan and thus a membership fee exists. This membership fee will not be billed to your insurance. The membership fee does not affect the co-payments, co-insurance or deductibles you are required to pay under the terms of your insurance coverage for covered services such as the above-mentioned office visit charges.
**All deductibles & copays apply to visits. Visits & labs are separate from membership payment. While we suggest and order clinically useful labs, the processing/insurance coverage/payments are separate from Mint Health. Mint Health is not responsible for insurance reimbursement for any labs or services. Patients are responsible for confirming lab coverage and costs with their insurance provider. We offer optional cash-pay pricing for transparency and affordability.
Mint Health Membership includes:
In addition to billing insurance for visits, membership fee is $2,200.00 per year for uncovered services (payment options: 1 time upfront, monthly payment or CareCredit options available)
Expanded visit time with providers for coaching needs and holistic approach to visits typically covered by your insurance using the FM Matrix.
Individualized and comprehensive plans to holistically address GYN focused concerns (see services list), hormone health, preconception & fertility with a body systems based approach as per the FM Matrix.
Specific plans to holistically address NON-GYN concerns (see services list)
Individual coaching on addressing important lifestyle and environmental factors for your health
Access to review of expanded blood lab panels for body systems, nutrients, immune regulation, inflammation, hormonal, metabolic assessments
Access to review of specialized Functional Medicine testing such as stool and urine for inflammation/intestinal permeability testing, toxin assessments, oxidative stress, mitochondria, mold testing and methylation testing
Annual health review with status update on health indices defined in the Functional Medicine (FM) Matrix (shown below)

As soon as your application is reviewed, we will reach out with the membership agreement and annual membership fee payment options.
Once your membership agreement and payment are received, we'll begin or transition your care process as outlined above.

You can schedule with either provider to get your answers and feel confident about whatever your decision may be prior to making a membership commitment. This is a clinical visit and will be billed to your insurance; you will be responsible for any copay, coinsurance, or deductible per your plan.
